29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) Jobsite - Employees were not protected from falling through skylights on the walking working surface while working on the roof. This condition exposed employees to a fall hazard of approximately ten (10) feet. Observed on or about 10 MAR 2015.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) Jobsite - Employees installing roof material on a low-slope roof were exposed to fall hazards of approximately twenty (20) feet without the use of conventional or alternative methods of fall protection. Observed on or about 10 MAR 2015. Union Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10) which was contained in OSHA inspection number 446753, citation number 1, item number 2 and was affirmed as a final order on 08/11/2012 with respect to a workplace located at 660 East Erie Avenue, Philadelphia, PA 19134.
