29 CFR 1926.701(b):     Reinforcing steel. All protruding reinforcing steel, onto and into which employees could fall, shall be guarded to eliminate the hazard of impalement.      Jobsite:  Employees were exposed to impalement hazards from #4 rebar that was not protected with rebar caps around the southeast corner of the project where employees were installing concrete wall forms on all four sides of the work zone. 29 CFR 1926.1053(b)(1):     When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.      Jobsite:  Employees working in an open excavation did not have a properly placed ladder that extended 3 feet above the upper level for easy access in and out nor was the angle of the ladder properly placed.

29 CFR 1926.651(c)(1)(v):     Structural ramps used in lieu of steps shall be provided with cleats or other surface treatments o the top surface to prevent slipping.      Jobsite:  Employees were acessing southeast work zone by using a 2x10 plank on one side(with no cleats to prevent slipping) which passed through and over a concrete footing and nothing used for the other side where employees had to step down greater than 19" inches to the other side exposing employees to a slip and fall.
