29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, equipment found to be damaged or defective was being used prior to being repaired: A) An extension cord in use was missing the ground pin which allowed it to be connected to another cord backward, reversing polarity to the tool. B) An extension cord in use had been improperly repaired with electrical tape.
