29 CFR 1910.132(d)(1):     The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):    (a) On or about 05/30/12, and at time prior thereto, the employer did not assess the lab after a major refurbisment of the work area in which hazardous chemicals are in use.

29 CFR 1910.132(a):     Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided when it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.      (a) At the San Antonio Military Medical Facility Dinning Hall, on or about 05/30/12, and at time prior thereto, employees were exposed to severe burns when a relief valve on an industrial cooking pot failed and was leaking during the inspection, resulting in an uncontrolled release of steam and boiling water in close proximity to kitchen staffs legs.

29 CFR 1910.22(a)(2):     The floor of every workroom was not maintained in a clean and, so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable.    (a) At the San Antonio Military Medical Facility Dining Hall, on or about 05/30/12, and at times prior thereto, employees were exposed to a slip hazard because the floor of the kitchen along the main line was not maintained in a clean and, so far as possible, a dry condition due to faulty valve.

29 CFR 1910.1030(h)(5)(i):     The employer did not establish and maintain a sharps injury log for the recording of percutaneous injuries from contaminated sharps    (a) At San Antonio Military Medical Center, on or about 05/30/12, and at time prior thereto, employees were exposed to future sharps injuries due to the employers lack of analysis of brand, department, and explanation of the incident.
