29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).    The employer does not fully plank each platform on all working levels of scaffold. This violation was observed at the construction site on May 29, 2012 where employees were exposed to a 12.5 foot fall hazard while working from a scaffold that was not fully planked.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    The employer does not protect from falls each employee working on scaffolds more than 10 feet above a lower level. This violation was observed at the construction site on May 29, 2012 where an employee was exposed to a 12.5 foot fall hazard while working from a scaffold without fall protection system.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.      The employer does not provide scaffolds with a proper ladder access. This violation was observed at the construction site on May 29, 2012 where employees were exposed to a 12.5 foot fall hazard while working from a scaffold that did not have proper access.

29 CFR 1926.452(a)(4): Diagonal bracing in both directions were not installed across the entire outside face of all double- and single-pole scaffolds.    The employer does not ensure that every pole scaffold has installed diagonal bracing in both directions across the entire outside face of the scaffold. This violation was observed at the construction site on May 29, 2012 where employees were exposed to a 12.5 foot fall hazard while working from a scaffold without diagonal bracing.
