29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide: The employer does not ensure all scaffold platforms are at least 18 inches wide. This violation was observed on or about October 17, 2012, at the construction site when an employee was exposed to a fall hazard while working on a scaffold platform that was approximately 10 inches wide. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all scaffolds platforms and walkways are at least 18 inches wide.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: The employer does not ensure that a guardrail system is installed on all working levels of scaffolds to prevent employee injury. This violation was observed on or about October 17, 2012, at the construction site when an employee was exposed to a fall hazard while on a working on a scaffold platform without a guardrail system in place. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all scaffolds used by employees have a guardrail system installed that protects employees from falling from all open sides and ends of the platform.
