29 CFR 1910.95(c)(1):     The employer shall administer a continuing, effective hearing conservation program, as described in paragraphs (c) through (o) of this section, whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response) or, equivalently, a dose of fifty percent. For purposes of the hearing conservation program, employee noise exposures shall be computed in accordance with appendix A and Table G-16a, and without regard to any attenuation provided by the use of personal protective equipment.    a) In the rear grinding room, an employee operating a gringer was exposed to an Action Level Dose of 61.22% or an equivalent dBA (decibels on the "A" scale) of 86.4, which was capable of causing hearing loss.  The sampling was performed for 441 minutes during one shift on May 18, 2012.    b) In the grinding area, an employee operating a grinder was exposed to an to an Action Level Dose of 180% or an equivalent dBA (decibels on the "A" scale) of 94.2 which was capable of causing hearing loss.  The sampling was performed for 463 minutes during one shift on May 31, 2012.    c) In the rear grinding room, an employee operating a grinder was exposed to an Action Level Dose of 94.71% or an equivalent dBA (decibels on the "A" scale) of 89.6, which was capable of causing hearing loss.  The sampling was performed for 331 minutes during one shift on May 31, 2012.    ABATEMENT CERTIFICATION IS REQUIRED
