29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): a) On or about May 31, 2012 and at times prior, employees were not protected from cave-in hazards while installing pipe in an unprotected trench. The trench was approximately 4 feet wide, had vertical walls and was greater 7 feet in depth in several areas where employees were working.
