29 CFR 1926.350(a)(10):     Oxygen cylinders in storage were not separated from fuel-gas cylinder or combustible materials (especially oil or grease), a minimum distance of 20 feet or by a noncombustible barrier at least 5 feet high having a fire-resistance rating of at least one-half hour.    At the site, an oxygen and acetylene cylinder had been lying on their side, together, for at least 2-3 days, exposing employees to explosion hazards.

29 CFR 1926.502(d)(15):     Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.    At the location, anchors and synthetic ropes were installed by employees who had not received training to do so and were directed by the supervisor (the qualified person), with ropes that were not in good condition, too long allowing the employee to hit the ground if a fall occurred, and employees were tied off to knots in the rope.

29 CFR 1926.760(a)(1):     Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.    A) At the site, employees were walking the beams to access the decked area to reach the ground and were exposed  to falls up to 25' or more.    B)At the site, employees engaged in installing insulation under the roof, as part of the series of work for erecting a pre-fabricated metal building, were exposed to fall hazards because, while they were given PFAS and synthetic ropes to tie off to, the ropes, when extended, came to approximately 6' above the ground.
