29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about December 16, 2021, and times prior thereto, at a construction site located at 13145 I-45, Willis, Texas, employees were exposed to fall hazards exceeding six feet when repairing leaks on the roof without a fall protection system.

29 CFR 1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. On or about December 16, 2021, and times prior thereto, at a construction site located at 13145 I-45, Willis, Texas, employees were exposed to fall hazards when using an portable ladder to access the roof without extending the ladder 3 feet beyond the landing of the roof.
