29 CFR 1910.333(c)(4)(ii): Where lack of illumination or an obstruction precludes observation of the work to be performed, employees may not perform tasks near exposed energized parts. Employees may not reach blindly into areas which may contain energized parts. Job site located at the Police Headquarters at 15th and Jackson, Omaha, NE-Employee was in the process of installing ceiling sensors for the lighting for the building. The junction box the sensors had to be added to had been installed incorrectly and the employee had to reach into the box blindly to locate the wire to install the sensor on. As he was in the process of unfolding the wires in the box he contacted a hot wire causing the electrical shock he received. The junction box was on a 20 Amp circuit/277 volts.

29 CFR 1910.333(b)(2)(iv)(B): Qualified person shall use test equipment: Job site located at 2121 S 24th Street, Omaha, NE 68108-One employee received an electrical shock while working on a junction box installing light sensors. The employee had not tested the circuits to verify that the circuits were isolated. Abatement certification is required for this violation. Abatement certification sheet is enclosed with the citation.
