OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in-that employee(s) were exposed to crushing hazards: (a) Building 42, West Bay North: An OHIO lifting magnet used to handle round corner square (RCS) bar did not have the rated load capacity, weight of lifting magnet, duty cycle and other required identification information marked/affixed to the lifting device. This exposed the employee(s) to a crushing hazard.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by unused portion of saw blade: (a) Kasto Horizontal Band Saw; Machine #71030748: Employees use a Kasto Horizontal Band Saw to cut RCS bar. The saw did not have the unused portion of the blade guarded, exposing the operator(s) to an amputation hazard.

29 CFR 1910.305(g)(1)(iv): Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv) (a) Building 42, Kasto Saw: A yellow 100 foot extension cord was used as fixed wiring. The extension cord was wired into a four gang outlet box and mounted to the saw control panel. The extension cord was run under the saw to the saw blade storage rack. The extension cord then was secured in place along the storage rack frame and run to an outlet box located under the HEM Saw. This exposed the cord to damage and exposed employees to an electrical shock hazard.
