29 CFR 1910.36(d)(1):     Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:    (a) On or about 6/5/12 the exit door leading out of the storage room from the rear of the shop was locked with a bolt with a padlock on it in addition to a thumb lock in the handle. Employees are exposed to the hazard of being trapped in the room in an emergency, such as but not limited to, fire.

(a) On or about 6/5/12 the employer did not provide an eyewash and shower for immediate emergency use. Access to the eye wash and shower was partially blocked by the water fountain and its location between the wall and paint sray booth. Employees are exposed to the hazard of chemical burns.

29 CFR 1910.253(b)(4)(iii):     Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.    (a) On or about 6/5/12 oxygen cylinders were not separated from fuel-gas cylinders (propane and acetylene) in the starage room in the rear of the shop. The company also stores oil and tires in the room. Employees are exposed to the hazard of smoke inhalation and burns from fire.
