29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents: (a) L & M Roofing at 4900 E. 100th Dr. in Thornton, CO: On June 6, 2012 the employer did not provide and require the use of eye protection when roofers were exposed to eye hazards in that the roofers were not wearing safety glasses when using nail guns. This exposed the workers to an eye hazard. L & M Roofing, Inc. was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926. 102(a)(1) which was contained in OSHA Inspection Number 314659657, Citation 1, Item 1, issued on October 14, 2010, and became Final Order on October 18, 2010 with respect to a workplace located at 15832 E. 98th Pl. in Commerce City, CO.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system: (a) L & M Roofing Inc. at 4900 E. 100th Dr. in Thornton, CO: On and before June 6, 2012, the employer did not ensure that employees were protected from injury in that the owner and six employees installing shingles on a 5:12 pitch roof were not utilizing fall protection systems. This condition exposed employees to a fall hazard of approximately 10 feet and more than 20 feet. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet"). L & M Roofing, Inc. was previously cited for violations of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926. 501(b)(13) contained in OSHA Inspection Number 417783 Citation 2, Item 1, issued on May 25, 2012, and became Final Order on May 30, 2012 with respect to a workplace loacted at 306 Collyer St. in Longmont, CO, and OSHA Inspection Number 314659657, Citation 1, Item 2, issued on October 14, 2010, and became Final Order on October 18, 2010 with respect to a workplace located at 15832 E. 98th Pl. in Commerce City, CO.
