29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    a) Therm-O-Disc, Incorporated. - Mansfield, Ohio: On or about June 6, 2012, the employer did not conduct annual audits of its energy control procedures to assure that proper lockout/tagout procedures and requirements were being followed.  Lack of procedure audits exposed employees to amputation injuries in that adequate machine isolation was not being followed when employees accessed hazard areas of presses during set-up.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:    a) Therm-O-Disc, Incorporated - Mansfield, Ohio: On or about February 6, 2012, the employer did not assure that employees who perform press set-up activities to the horizontal mechanical power press #5 were trained in the skills required for the safe application, usage, and removal of the energy controls.  Operators that service the machine was not trained to lockout the machine to protect against pinch point hazards.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):    a) Therm-O-Disc, Incorporated - Mansfield, Ohio: On or about February 6, 2012, the employer did not assure that energy isolating devices to control the energy of the horizontal mechanical power press #5 were operated so as to isolate hazardous energy sources while employees conducted press set-up activities.  Employees that serviced the press were exposed to amputation and crushing injuries.      b) Therm-O-Disc, Incorporated - Mansfield, Ohio: On or about June 6, 2012, the employer did not assure that energy isolating devices to control the energy of the horizontal mechanical power press #5 were operated so as to isolate hazardous energy sources while employees conducted die change/set-up activities.  Employees that accessed the hazard areas of the press were exposed to crushing and amputation injuries during press servicing tasks.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Employees were not protected through the use of adequate machine guarding or the isolation of  the energy sources when setting up the Chapee #5 horizontal mechanical power press.    b) Therm-O-Disc, Incorporated - Mansfield, Ohio: On or about June 6, 2012, the employer did not assure die setting procedures were established which would protect employees from pinch points.  Employees were not protected through the use of adequate machine guarding or the isolation of  the energy sources when setting up the Chapee #5 horizontal mechanical power press. 29 CFR 1910.217(f)(2): Supervision to ensure that correct operating procedures were being followed on mechanical power press(es) was inadequate:    a) Therm-O-Disc, Incorporated - Mansfield, Ohio: On or about June 6, 2012, the employer did not assure that the operator(s) of the horizontal mechanical power press #5 was instructed in the safe operation of the press in that the key selector switch for the press run mode was not supervised.  The key, used to change press modes, was kept in the selector switch and not supervised to assure that the press mode was not changed without the operators knowledge.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
