29 CFR 1910.217(e)(1)(i)(A): Periodic and regular inspections of each power press were not conducted to ensure that all of its parts, auxiliary equipment, and safeguards, including clutch/brake mechanism, antirepeat feature, and single-stroke mechanism, are in safe operating condition and adjustment:         (a)  Therm-o-Disc, Inc., 1320 S. Main Street, Mansfield, Ohio:  On or about May 6, 2015, the employer did not conduct periodic and regular inspections of the U.S. Baird mechanical power press #17 to ensure that the brake system was operating safely.  When applied, the brake did not prevent the ram from moving downward.  Employees were exposed to caught-in hazards and at risk of serious injury including but not limited to crush and amputation injuries.

1910.217(e)(1)(i)(B): The employer did not perform and complete necessary maintenance or repair, or both, before operating the press:           (a)  Therm-o-Disc, Inc., 1320 S. Main Street, Mansfield, Ohio:  On or about May 6, 2015, the employer did not perform and complete necessary maintenance on the brake system of the U.S. Baird mechanical power press #17.  The press was in service with a brake that, when applied, did not prevent the ram from moving downward.
