29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):     a- SE Corner of 1600 S. Camac St, Tubular Welded Frame Scaffold-Employees were exposed to fall hazards of approximately 19 to 24 feet where the active work platforms were not fully planked between the front uprights and the back guardrail supports.   Observed 6/5/2012    NOTE: Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph.    a- SE Corner of 1600 S. Camac St, Tubular Welded Frame Scaffold- Employees were exposed to fall hazards of approximately 24 feet where the scaffold was built to an elevation greater than the maximum allowable 4:1 height to base ratio with no guying, tying or bracing installed.  The scaffold was 5 feet in depth.   Observed 6/5/2012.     NOTE: Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6):     a) SE Corner of 1600 S. Camac St, Tubular Welded Frame Scaffold- The scaffold was built around an energized (240 VAC) Tri-plex type power line feeding the structure.  The insulated power line was within inches of the scaffold with employees having to step over it.  The insulated power line was required to be at least three (3) feet away from the scaffold.  Observed 6/5/2012.     NOTE: Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:     a) SE Corner of 1600 S. Camac St, Tubular Welded Frame and Ladder Jack Scaffolds- The scaffolds were not erected under the supervision of a competent person.  Numerous deficiencies such as, but not limited to lack of fall protection, lack of adequate planking, power lines not protected, and lack of guying, tying, and/or bracing existed with the scaffolds.  Observed 6/5/2012.     NOTE: Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Camac St, Ladder Jack Scaffold- An employee was exposed to a fall hazard of approximately 20 feet while working from the ladder jack scaffold and not utilizing the required personal fall arrest system.
