29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items A through D of this section.    (a)  Facility Wide - Employees perform maintenance and servicing on equipment such as but not limited to the PRD and Driam with energy sources such as electrical and hydraulic.  The employer did not develop clear energy control procedures, on or about June 6, 2012.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.305(g)(2)(ii): Flexible cords were not used in continuous lengths without splice or tap:    (a)  PRD Welding Control Box - When employees welded steel rolls together they used a control box that contained a splice wire.  The employer did not ensure that flexible cords were used in continuous lengths, on or about June 6, 2012.    No abatement certification or documentation required.
