29 CFR 1915.506(b)(1): Before any work was done in a space equipped with fixed extinguishing systems the employer did not physically isolate the fixed extinguishing systems or utilize other positive means to prevent the systems discharge: In the water maker room and adjacent protected areas on the vessel Noble Sam Noble, located on Keppel AmFELS, LLC dry dock, employees were engaged in repair and maintenance activity. The employer did not ensure the carbon dioxide fire suppression system was secured and/or isolated, exposing employees to an asphyxiation hazard.

29 CFR 1915.506(d)(2): The employer did not ensure all inward opening doors, hatches, scuttles, and other potential barriers to safe exit were removed, locked open, braced, or otherwise secured so that they remain open and accessible for escape if systems' activation could result: In the water maker room and adjacent protected areas on the vessel Noble Sam Noble, located on Keppel AmFELS, LLC dry dock, employees were engaged in repair and maintenance activities. In the following areas, the employer did not ensure employees had clear, secured, unimpeded, safe passageways and exits in the event of a carbon dioxide release: a. Engine room, in front of EMD engine #1, barricade scaffolding over floor hole obstructing passageway. b. Engine room, between engines, scaffolding utilized to access the ceiling plenum obstructing passageway. c. OmniPure room, doorway between engine room and OmniPure room, scaffolding obstructing passageway/exit. d. The door between the water maker room and the engine room was not physically secured from closing.
