29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by pressurized molten metal: On or about May 24, 2012, in the Magnesium Department, on the Idra DCM #43, 300 ton hot chamber die cast machine, the guard had openings between the guard and machine when closed, which allowed the pressurized molten metal to spray out onto employees when the goose neck that fed the die cracked.
