29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the following elements and actions and was not done in the following sequence: On and before August 4, 2015, employees conducting service and maintenance activities, such as, but not limited to, unplugging the funnel, waxing the die, and checking the spray, on DCM cells 55 and 9, did not cover the elements and actions in sequence as outlined in 29 CFR 1910.147(d)(1) through (d)(5). The employer did not provide effective alternative measures to ensure protection from the hazardous energy in the cell.
