The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: (a) Energy control procedures (LOTO procedures) were not utilized during the set-up process of equipment such as but not limited to: W/C 5 Wegner Press exposing employees to unexpected start-up/energization of the equipment.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) At W/C 53, a Roller (1/2 HP motor) used to roll steel plate had inadequate guarding. Employees were exposed to caught-in hazards. (b) At W/C 10, A Bruck Shear had inadequate guarding when cutting/shearing the sheets of metal on the outfeed portion of the shear exposing employees to struck-by hazards. (c) At W/C 12, the Strippit Turret Press hood would be in the up position exposing employees to struck-by hazards from the part coming out. (d) At W/C 24, the Bruck Shear had inadequate guarding on the outfeed section in that the guarding (hinged) allowed access to the point of operation from the side and underneath exposing employees to struck-by hazards. (e) At W/C 20, the Wegner Part Rev press with shear had inadequate guarding on the outfeed section, in that the guarding allowed access to the point of operation from underneath exposing employees to struck-by hazards.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): (a) At W/C 21 by the Kumla the pendant for the ZipII under hung chain hoist had exposed outer sheeting pulled from the wires exposing employees to electrical shock hazards.
