29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Remlinger Manufacturing Company, Inc. - Kalida, Ohio: On or about June 7, 2012, employees operating the Okuma drilling machine, model MC-4VA (s/n 6102-0527) were exposed to injuries associated with the machines rotating parts.  The employer did not assure the rotating parts were guarded.    b) Remlinger Manufacturing Company, Inc. - Kalida, Ohio: On or about June 8, 2012, employees operating the Okuma machining center #501 were exposed to injuries from the machines rotating parts, pinch points, and/or flying chips.  The employer did not assure that the machine was adequately guarded to prevent employees from accessing the hazard area during operation.      c) Remlinger Manufacturing Company, Inc. - Kalida, Ohio: On or about June 8, 2012, the employer did not assure that the Rockwell drill press located adjacent to the #501 machining center, was guarded to protect employees from its rotating parts.    d) Remlinger Manufacturing Company, Inc. - Kalida, Ohio: On or about June 8, 2012, employees operating the Giddings & Lewis Numeridrill (#370) were exposed to injuries associated with the machines rotating parts.  The employer did not assure the rotating parts were guarded.    e) Remlinger Manufacutring Company, Inc. - Kalida, Ohio: On or about June 26, 2012, the employer did not assure that the point of operation of the #372 Amada press brake was guarded to protect operators from amputation injury.    f) Remlinger Manufacutring Company, Inc. - Kalida, Ohio: On or about June 26, 2012, employees operating the Motoman robot cell were exposed to struck-by injuries from moving components of the robot in that perimeter guarding did not adequately protect employees.    g) Remlinger Manufacturing Company, Inc. - Kalida, Ohio: On or about June 26, 2012 employees operating the Fanuc Arcmate 100i front robot were exposed to point of operation and struck-by injuries from moving components of the robot in that the equipment was not adequately guarded to protect employees.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
