29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: (a) At the side of the building facing the Autoport on or about June 7, 2012 and at times prior thereto, an employee was using an extension cord with a missing ground pin to provide temporary electrical service to a Makita HM1201 Demolition hammer. The Demolition hammer was used to chip the ground in order to make a 12 inch deep trench for underground utilities.
