29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    Weld Shop -  On 6/7/12, and at times prior thereto, a Baldor bench grinder, SN# F28736, had two tongue guards that were not adjusted to within 1/4 inch of the grinding wheel.  The gap on the right side was 3/4 inch and the gap on the left side was 1 inch.
