29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, and enclosures was not permanent and continuous: (a) On the east side of the second level addition on or about June 11, 2012, and at times prior thereto there was no grounding pin on a plug for a 110 VAC floor standing metal fan. (b) On the west side of the second level addition on or about June 11, 2012, and at times prior thereto there was no grounding of the metal fixture that contained two, 110 VAC four foot long florescent light bulbs. The fixture that was mounted to a wooden stand, and the top of the fixture was 64 inches from the floor. The grounding wire was cut off where the cable entered the fixture.

29 CFR 1926.405(a)(2)(ii)(E): All lamps for general illumination were not protected from accidental contact or breakage: (a) On the west side of the second level addition on or about June 11, 2012, and at times prior thereto two, four foot long florescent light bulbs were not protected from accidental contact or breakage as there was no cover or screen over the bulbs. The bulbs were in a fixture that was mounted to a wooden stand. The top of the fixture was 64 inches from the floor.

29 CFR 1926.405(b)(2): All pull boxes, junction boxes, and fittings were not provided with covers. When metal covers are used, they were not grounded. In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass were not provided with bushings designed for the purpose or they did not have smooth, well-rounded surfaces on which the cords may bear: (a) On the west side of the second level addition on or about June 11, 2012, and at times prior thereto there was no bushing or well rounded surface where the power cord enter the metal fixture that contained two, 110 VAC four foot long florescent light bulbs. The fixture that was mounted to a wooden stand, and the top of the fixture was 64 inches from the floor.

No operator was in site. The motor was off.
