29 CFR 1910.178(l)(1)(i):  The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).    a)  On or about June 4, 2012 and at times prior thereto, technicians used a Toyota powered industrial truck to lift air conditioning units on top of recreational vehicles for installation without being trained or evaluated.    Abatement was completed for this item.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not conducted at least once every three years.    a)  On or about June 4, 2012, and a times prior thereto, technicians used a Toyota powered industrial truck to lift Air Conditioning units on top of Recreational Vehicles for installation without completing powered industrial truck refresher courses every three years.    Abatement was completed for this item.

29 CFR 1910.213(d)(1): Each circular crosscut table saw was not be guarded by a hood that completely enclosed the portion of the saw above the table and that portion of the saw above the material being cut.     a)  Woodworking Area:  On or about June 4, 2012, and at times prior thereto, the Delta Table Saw had been used for cutting wood trim for recreational vehicles without an overhead guard.    Abatement certification is required for this item.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.    a)  On or about June 4, 2012, and at times prior thereto, the employer had not certified that a workplace hazard assessment was performed to determine the personal protective equipment required in the workplace.   Techicians in the facility perform tasks such as welding, refilling propane bottles, and work from heights over 10 feet.    Abatement certification is required for this item.

29 CFR 1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use Personal Protective Equipment.    a)  On or about June 4, 2012, and at times prior thereto, employees are required to perform maintaining and servicing activities on top of recreational vehicles and trailers that are over 10 feet tall without training on the use of the fall protection systems.    b)  On or about June 4, 2012, and at times prior thereto, employees are required to refill propane containers at an Amerigas servicing station without training regarding the use of eye protection and gloves during this activity.    Abatement was completed for this item.

29 CFR 1910.215(a)(4):  Work rests were not adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage.     a)  Metal Shop:  On or about June 4, 2012, and at times prior thereto, a work rest on the Baldor Bench Grinder was one inch from the abrasive wheel.    b)  Metal Shop:  On or about June 4, 2012, and at times prior thereto, a work rest on the Baldor Bench Grinder was three-quarters of an inch from the abrasive wheel.    Abatement certification is required for this item.
