On or about April 13, 2012, employees working on the roof of the three story apartment building were not utilizing fall protection to protect from falling 30 feet to the ground. b) construction site, 4400 East Lake Rd., Erie, PA. On or about April 13, 2012, employees constructing the decking above the first level of the building, 10 feet above the ground, were not utilizing fall protection. c) construction site, 4400 East Lake Rd. in Erie, PA. On or about April 13, 2012, employees working on balconies were not utilizing fall protection to protect from falling 20 to 30 feet to the ground.

On or about April 13, 2012, employees were using self-supporting, A-frame ladder while still collapsed to conduct work on the third story balconies of the apartment building. b) construction site, 4400 East Lake Rd., Erie, PA. On or about April 13, 2012, employees were using a self-supporting, A-frame ladder to access the level above the first floor by stepping off the the top rung and climbing onto the working level.
