29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:   a) New Hartford Senior Housing Complex - On or about 5 November, 2013 - Employees were using an extension cord to power a circular saw that had no strain relief to prevent pull from being directly transmitted to conductor connections.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) New Hartford Senior Housing Complex - On or about 5 November, 2013 - Employees were working on the roof of the new residential housing complex without the use of a fall protection system.   Saldano Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 901994, citation number 02, item number 001 and was affirmed as a final order on 24 June, 2013, with respect to a workplace located at Morgan Square Senior Apartments, Clay, NY.  Saldano Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 463858, citation number 01, item number 001 and was affirmed as a final order on 22 October, 2012, with respect to a workplace located at 4400 East Lake Road, Erie, PA.

29 CFR 1926.1053(b)(1):  Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   a) New Hartford Senior Housing Complex - On or about 5 November, 2013 - Employees were using a portable ladder to access the roof of the building that did not have side rails extended a minimum of 3 feet above the landing surface.    Saldano Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 930003, citation number 01, item number 002 and was affirmed as a final order on 21 September, 2013, with respect to a workplace located at 1600 Dickson Avenue, Scranton, PA.
