29 CFR 1926.405(b)(2): All pull boxes, junction boxes, and fittings were not provided with covers. (a) On a fourth level interior wall on or about June 11, 2012, and at times prior thereto, there was no cover on a panel box that contain electrical equipment energized at 110 VAC and 220 VAC. The panel box was 21 inches high by 14 inches wide, and the bottom was 52 inches from the floor. Employees carried metal shores nearby.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) On the east side of the roof on or about June 12, 2012, and at times prior thereto, there was no fall protection for employees installing rebar. A 2 foot wide concrete, horizontal awning for rain and sun protection was located about 9 feet, 4 inches below the roof. The roof was 37 feet, 4 inches above the ground below.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: (a) Near the front exterior of the building on or about June 12, 2012, and at times prior thereto, eye protection was not used while using a drill to secure metal studs to the building. Cement boards were to be attached to the studs for stonework.
