29 CFR 1928.57(a)(6): The employer did not instruct every employee in the safe operation and servicing of all covered equipment with which he was involved at least annually:    On or about June 11th, 2012 employees operating equipment were not instructed in the safe operation and servicing of all covered equipment. Employees did not identify when power take-off guarding was inadequate on two John Deere tractors and feed mixers.     Abatement Certification and Abatement Documentation Required.

29 CFR 1928.57(b)(1)(i): All power take-off shafts, including rear, mid- or side-mounted shafts, shall be guarded either by a master shield, as provided in paragraph (b)(1)(ii) of this section, or by other protective guarding.    On or about June 11th, 2012 two John Deere covered tractors powering feed mixers did not have adequate guards covering the power take-off shafts behind the tractors.     Abatement Certification and Abatement Documentation Required.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (Agriculture Reference: 1928.21(a)(5))    On or about June 11th, 2012 the employer did not develop a written hazard communication program where employees had exposure to and used hazardous chemicals such as acid concrete cleaner, teat dip and/or polyacrylamide copolymer.
