At the time of initial assignment and at least annually thereafter, the employer shall instruct every employee in the safe operation and servicing of all covered equipment with which he is or will be involved, including at least the following safe operating practices: a) On or about January 16, 2015, during servicing and maintenance activities on the Meyer's Heavy Duty Manure Spreader, located in the yard outside the mechanic shop, the employer did not ensure that the John Deer 8410 tractor's engine was stopped, the power source was disconnected, and all machine movement had stopped before servicing or maintenance was performed on the Meyer's Heavy Duty Manure Spreader.

29 CFR 1910.1200(f)(5): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein: a) On or about January 16, 2015, during oiling activities for the crowd gate the little container with oil, located in the pasteurization room next to the Dairy Parlor in the in the new site, the employer did not ensure that oil container was labeled, tagged or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein. b) On or about January 16, 2015, during oiling activities for the hydraulics for the rotary parlor, there were two 2 gallon containers with used petroleum oil, located in the basement underneath the Dairy Parlor in the in the new site, the employer did not ensure that oil container was labeled, tagged or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein. c) On or about January 16, 2015, during oiling activities for the hydraulics for the rotary parlor, the 15 gallon container with used petroleum oil, located in the basement underneath the Dairy Parlor in the in the new site, the employer did not ensure that oil container was labeled, tagged or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein.
