29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.     The employer does not ensure that each scaffold platform is fully decked between the platform and uprights so that no more than a 1 inch gap remains. This violation was observed on June 11, 2012, on the east side of a newly built structure where employees were exposed to 12 and 18 foot fall hazards while installing metal lath from a set of fabricated frame scaffolds that was planked half of the width leaving a gap of approximately 28 inches.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    The employer does not ensure that scaffold platforms more than 2 feet above or below a point of access are provided with a ladder or other means of access. This violation was observed on or about June 11, 2012, on the east side of a newly built structure where employees were exposed to a 12 and 18 foot fall hazard while accessing a set of fabricated frame scaffolds that did not have a ladder or other means of safe access.

This violation was observed on June 11, 2012, on the east side of a newly built structure where employees were exposed to 12 and 18 foot fall hazards while installing metal lath from a set of fabricated frame scaffolds that did not have braces installed on all sets.

This violation was occurred on or about June 11, 2012, or times prior thereto, where employees on the east side of the newly built structure were using a Chicago Electric 1/2 inch drill that had a frayed electrical cord.
