29 CFR 1917.45(d)(2): The employer used a crane or derrick having a visible or known defect affecting safe operation. a) StarKist Dock, flatbed crane #6. On 08 May 2012, and at times prior thereto, crane #6 while unloading the vessel Koorales had an oil leak from the bottom of the structure. The oil spilled on the dock exposing workers to a fire hazard and a tripping hazard.

No quadrennial inspection records were available to show and verify that the cranes 5 & 6 were inspected 4 years ago.
