On 08 May 2012, and at times prior thereto, thorough inspections of chain slings were not conducted quarterly and each chain sling was not marked (painted or color coated) to show when it was last inspected or the month inspected.

On 08 May 2012, and at times prior thereto, chain slings for hoisting baskets and nets full of fish from the vessel Koorales was not marked to identify the size, grade and the rated capacity of the chain slings.

29 CFR 1917.45(d)(2): The employer used a crane or derrick having a visible or known defect affecting safe operation. a) StarKist Dock, flatbed crane #6. On 08 May 2012, and at times prior thereto, crane #6 while unloading the vessel Koorales had an oil leak from the bottom of the structure. The oil spilled on the dock exposing workers to a fire hazard and a tripping hazard.

No quadrennial inspection records were available to show and verify that the cranes 5 & 6 were inspected 4 years ago.
