1926.452(w)(5) Scaffolds were not stabilized to prevent tipping during movement: (a) At the project on or about June 13, 2012, and at times prior thereto, the JLG 2630ES, Serial Number 0200127245, was not stabilized to prevent tipping during movement. The front left tire was worn and bits of rubber was missing thereby allowing the rim to make contact with the floor surface.

29 CFR 1926.50(d)(2): The contents of the first-aid kit were not checked by the employer before being sent out on each job or at least weekly on each job to ensure that the expended items were replaced: (a) At the project on or about June 13, 2012, and at times prior thereto, the first aid kit was not checked by the employer prior to being sent out on each job and at least weekly on each job to ensure that the expended items are replaced. A one ounce bottle of Sterile Isotonic Solution within the first aid kit expired on March 2012.
