29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    (a) At the front side of the building on or about June 14, 2012, eye protection was not worn while painting overhead with a roller.  The paint was Dura Craft Exterior Acrylic Latex, # 6503-52156.  The warning label on the container said in the event of eye contact to flush eyes with large amounts of water and to get medical attention.  The MSDS states to wear safety spectacles with unperforated side shields.    (b) Near the south side of the building on or about June 14, 2012, gloves were not worn when a trowel was used to level concrete for the rainwater catch basin pad.  The concrete contained Portland cement.
