(a)(LOCATION)(IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)(DESCRIBE HAZARD(S) WHERE NECESSARY) At the job site located at 4800 Reavis Barracks Road, Affton MO, on 5 June 2012 the employer did not ensure employees wore safety glasses while they used nail guns. As a result the employees were exposed to hazards to the eyes. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: (a)(LOCATION)(IDENTIFY SPECIFIC EQUIPMENT AND/OR CONDITIONS)(DESCRIBE HAZARDS WHERE NECESSARY) At the job site located at 4800 Reavis Barracks Road, Affton MO, on 5 June 2012 the employer did not protect workers from the potential for accidental contact with energized power transmission lines. As a result employees engaged in installing the new roof worked within one foot of the power supply for the home. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).

The employer at the job site located at 8400 Reavis Barracks Road, Affton MO, on 5 June 2012, did not provide a fall protection system for the employees engaged in roofing construction. As a result the employees were exposed to falls of approximately 12 feet to the ground. Abatement certification and abatement documentation is required for this violation.
