29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person:  (a) Frequent and regular inspections of the job site, materials and equipment to be made by a competent person were not made as indicated by:          1.  Roofing work occurred  without fall protection.          2.  Workers working in the area below roofing work overhead were not wearing head protection.

29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a)  At the 2501 N 11th work site workers performing cleanup immediately below roofing operations were not wearing head protection to protect them from falling objects or debris.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) Workers were not trained to use fall protection prior to beginning to remove and or install roofing material.

29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a)  At the 2501 N 11th Street work site workers were on a steep roof performing roofing tasks without the use of fall protection.  American National Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13),  which was contained in OSHA inspection number 452896, Citation number 1   Item 2, and was affirmed as a final order on June 19, 2012 with respect to a workplace located  at 14806 Mondoubleau Lane, Florissant, MO 63033.
