29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: a) Front side of building: On June 7, 2012 and at times prior thereto each employee performing roofing activities on a 6 in 12 pitched roof, wore a personal fall arrest system with an adjustable lanyard attached to the front webbing of the harness. Note) Abatement certification IS required for this item.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used: a) Commercial roofing jobsite: On June 6, 2012 and at times prior thereto employees and the responsible person on site did not recognize and take action to minimize the hazard of having a horizontal rope lifeline attached to the webbing at the front of a harness when used as part of the fall protection system that was in place. b) Commercial roofing jobsite: On June 6, 2012 and at times prior thereto employees and the responsible person on site did not recognize and take action to minimize the potential hazards of using equipment that was not designed to be used for fall protection as part of the fall protection system that was in place. Equipment used included but was not limited to Petzl descenders and rapelling carabiners. c) Commercial roofing jobsite: On June 6, 2012 and at times prior thereto employees and the responsible person on site did not recognize and take action to minimize the hazard of not maintaining the proper horizontal rope lifeline adjustment to ensure effective fall restraint when used as part of the fall protection system that was in place. Note) Abatement certification IS required for this item.
