29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a) On or about November 2, 2021; employees were exposed to falls of approximately 35 feet while performing roofing activities including shoveling and transporting rock with a motorized hopper, to a chute located at the edge of the roof. 29 CFR  1926.502(d)(6)(v):A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself.  a) On or about November 2, 2021; a rope being used as part of a personal fall arrest system had been looped over a pipe being used as an anchor, and latched onto itself. 29 CFR  1926.502(d)(15):  Anchorage points used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds (22.2 kN) per employee.  a) On or about November 2, 2021 a wire anchor being used as part of a personal fall arrest system had been secured to a 2", galvanized steel pipe without knowledge if the pipe could withstand 5000 pounds of force. 29 CFR  1926.502(f)(1)(ii):29 CFR 1926.502(f)(1)(ii):  When mechanical equipment was being used, the warning line was not erected at least 6 feet (1.8 m) from the roof edge which is parallel to the direction of mechanical equipment operation, and at least 10 feet (3.1 m) from the roof edge which is perpendicular to the direction of mechanical equipment operation.  a) On or about November 2, 2021; an employee was operating a Whiteman MQ,  WBH-16 power buggy near a warning line that was set within 1 foot of the edge of the roof.
