29 CFR 1926.451(b)(1):     Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide, and the employer could not demonstrate that a wider space was necessary:      The employer does not ensure that the scaffold was fully planked during masonry work.  This violation was observed on or about June 12, 2012., on the north side of a newly built home where employees were exposed to a fall hazard when performing brick masonry work without a fully planked scaffold decked between uprights and guardrail supports.

29 CFR 1926.451(e)(1):   When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.    The employer does not provide employees with access to the scaffold. This violation was observed on or about June 12, 2012, on the north side of a newly built house where employees were exposed to a fall hazard of approximtely 16 feet when accessing the platform without a ladder.

29 CFR 1926.451(g)(4)(i):     Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:              The employer does not install guard rails on the scaffold.  This violation was observed on or about June 12, 2012, on the east of a newly built house where employees were exposed to a fall hazard when required to conduct brick masonry from a platform of 16 feet without a guard rail system.
