Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) Roof: On or about 03/04/2026, the employer exposed an employees to fall hazard of approximately 21 feet, 10 inches to the lower ground level, in that a fall protection system was not provided to an employee engaged in roofing activities.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Roof: On or about 03/04/2026, the employer exposed employees to fall hazards to the lower ground level, in that the side rails of the ladder did not extend at least 3 feet past the roof landing and was not secured to prevent accidental movement during use while engaged in roofing activities.
