29 CFR 1910.178(f)(2):  The storage and handling of liquefied petroleum gas fuel was not in accordance with NFPA No. 58-1969, Storage and Handling of Liquefied Petroleum Gases. Section 6.21.3.4(B): Fuel containers shall be mounted to prevent jarring loose and slipping or rotating, and the fastening shall be designed and constructed to withstand, without permanent visible deformation, static loading in any direction equal to four times the weight of the container filled with fuel:    At 7215 SW Topeka Blvd, Bldg 3A in the warehouse, forklift #366's propane tank was not secured, exposing employees to an explosion and fire hazard.

29 CFR 1910.303(b)(2):     Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    At 7215 SW Topeka Blvd, Topeka, KS, building 3A in the warehouse, electrical equipment was not being used in accordance with its design, exposing employees to potential shock and fire hazard in the following instances:    1) Electrical romex was being used to power electrical fixtures on the east wall without being enclused in conduit to protect it's outer sheathing.    2) Electrical romex wiring was spun around a 2"x4" on the east wall.    3) Light fixture was not secured and was being used as a portable light.

29 CFR 1910.305(j)(1)(i):     Fixtures, lampholders, lamps, rosettes, and receptacles located had live parts normally exposed to employee contact:    At 7215 SW Topeka Blvd, Topeka, KS, building 3A in the warehouse on the east wall, electrical equipment had exposed contacts, exposing employees to potential shcok hazards in the following instances:    1) A single-bulb light fixture located on the east wall of the warehouse was missing a light bulb and was wired for operation.    2) A light switch in the employee break area did not have a cover plate.    3) An electrical outlet in the employee break area did not have a cover plate.
