29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: a) At underside of the 690 overpass on North Crouse Avenue, on or about 5/21/12: Employees dis-assembled a 40 foot extension ladder and used each part of that ladder as separate ladders. One employee was injured when the section he was using (without "feet") slid out from underneath him and caused him to fall approximately 20 feet to the ground. Abatement certification must be submitted for this item.
