29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds per employee attached. a) At the jobsite, on or about 7/13/18: Workers were involved in pouring and finishing 7 pads each, onto 2 bridge abutments (14 total pads), between 16 and 21 feet to ground (varied grading), where they described using rebar to attach fall protection lanyards, and where that rebar was not designed for fall protection use, could not be confirmed to support at least 5,000 pounds, and was not designed as fall protection by a competent person.
