OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to crush and struck-by hazards created by unsafe operation of forklifts:    a) Facility - The three diesel-powered Hyster forklifts did not have seatbelts installed to restrain workers inside the cabin in case of tip-over.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.303(b)(7)(iv):     There were damaged parts that could adversely affect safe operation or mechanical strength of the equipment, such as parts that were broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating:    a) Plant -  The "Frog Pond" water bath was connected to a metallic 110 V quadruplex outlet box. The  plastic receptacles were broken and live parts were exposed.    b) Plant -  "The Frog Pond" water bath was connected to a 110 V metallic quadruplex receptacle box.  The male plug in the power cord was damaged. The plastic cover was loose and live parts were exposed.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

The box was not mounted, and was attached to a flexible cable in lieu of an extension cord.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.305(b)(1)(ii):     Unused openings in boxes, cabinets, or fittings were not effectively closed:    a) Plant - The 240 V Westinghouse panel board labeled "Panel H" had two unused openings with exposed live parts.    Kismet Rubber Products Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(b)(1)(ii), which was contained in OSHA inspection number 314207523 (373349), citation number 1, item number 3, issued on 04/20/11, case closed on 06/2011 with respect to a workplace located at 215 Industrial Blvd., Blue Ridge, GA 30513.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
