29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):   a) Plant - On or about 02/24/15 and at times prior thereto employees operated extruders including (but not limited to) extruder #5. The feed port was not guarded, and employees were exposed to amputation hazards.    Kismet Rubber Products Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 942851, citation number 1, item number 2, issued on 03/07/2014, and affirmed as a final order on 03/31/2014, with respect to a workplace located at 215 Industrial Blvd., Blue Ridge, GA 30513.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
