29 CFR 1926.300(b)(4)(ii):The point of operation of machines whose operation exposes an employee to injury, were not guarded:      (a) In the office hut onsite on or about June 18, 2012, and at times prior thereto, there was no point of operation guard for the blade on an Ace Hardware Professional 10 Miter Saw, model number 20624779C.  The saw was used to cut wood for the formwork, scaffolding, and stairway.

29 CFR 1926.452(a)(3): Diagonal bracing in both directions was not installed across the entire inside face of double-pole scaffolds used to support loads equivalent to a uniformly distributed load of 50 pounds (222 kg) or more per square foot (929 square cm):      (a) In the dining room and kitchen areas inside the house on or about June 18, 2012, and at times prior thereto, there was no diagonal bracing in both directions along the entire inside face of the double pole scaffold.  The platform was 12.5 feet high and about 40 feet long.  Employees work from the scaffold to pour concrete.

The top of the stairs was 6 feet high, and the bottom of the stairs was 4 feet from from the wall.

Open conductors did not conform to the minimum clearances of 10 feet (3.05 m)-above finished grade, sidewalks, or from any platform or projection from which they might be reached:      (a) Near the east side of the house under construction on or about June 18, 2012, and at times prior thereto, the non metallic sheath conductors suspended on poles were not at least 10 feet about the ground as they were 6 feet above the ground.  The conductors supplied 110 VAC to temporary outlets.
