29 CFR 1910.334(a)(3)(ii): Attachment plugs and receptacles were connected or altered in a manner which prevented proper continuity of the electric equipment grounding conductor at the point where plugs were attached to receptacles: In the Press Room at this location, employees operate a laundry press machinery where the grounding conductors had been modified exposing employees to electric shock such as but not limited to the following: 1. The a shirt and collar press machine had been connected to extension cord where its configuration did not accept a plug with a ground pin. 2. On the dry cleaning press, the ground pin had been removed (altered) from the plug end of its flexible cord.

The fan installed in the north wall had the conductor spliced and the ground wire was not reconnected and the outer sheath was missing. b. On the Dry Cleaning Press flexible cord and plug had been spliced, wrapped with black tape and bare copper conductor wires were exposed.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: At this location, employees engaged in laundry operations were exposed to hazardous chemicals such as but not limited to, Drirex, Picrin, Sofspot, StreePro, and Exit Dry Cleaning Fluid. The employer had not developed a program which would address labeling and other forms of warning on chemical containers, Material Safety Data Sheets and employee information and training on the hazards associated with chemicals used at this site. The written program must also contain the following: 1. a list of all hazardous chemicals on site. 2. the methods the employer will use to inform employees of the hazards associated with non-routine tasks involving chemicals, such as a spill; 3. the hazards of chemicals contained in piping that is not labeled and; 4. the method the employer will use to inform other employers (contractors) of the chemicals their employees might be exposed to while performing duties at this site.
