Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. This violation recently occurred on June 11, 2012, at 406 Woodlake Dr, Allen, TX; where employees were installing roofing tiles on a single family residence without using any type of fall protection.
